Step 01 - Go To 123FormBuilder and Create A Free Account

Step 02 - Go to your DASHBOARD and click "NEW FORM"

Step 03 -  Choose "BLANK FORM"

Step 04 - First click "NAME"

Click every option to "EDIT"

Then click "EMAIL"

 And  click "LONG TEXT" for Message Box

Step 05 - Now ready to Publish just click on the "PUBLISH" button

Step 06 - Here you can use Link or Embed file, It depends on you. Select and "COPY"

 Step 07 - Now go to your Blogger Dashboard and click "LAYOUT"

 Step 08 - Choose where are you want to add "CONTACT PAGE". Here I have selected "Footer Menu" and Click "SAVE ARRANGEMENT"

Step 08 - Or: 

Click "ADD A GADGET"

Then click the "HTML/JavaScript - Plus Button"

Paste here Your Contact Page Embed Code that you create in 123formbuilder.

And Save them.

And Finally, click " SAVE ARRANGEMENT"
